Dr. Patterson is available to see patients in office Monday, Wednesday, Friday  7:30am – 5pm.

We are usually available to see patients either the same or next day for urgent problems.

Dr. Patterson performs surgery on Tuesdays at the Day Surgery Center in Winter Haven and Thursdays at Winter Haven Hospital.

The health portal allows electronic communication between you and your providers. This internet-based patient portal is a secure, confidential and easy-to-use website giving patients 24-hour access to review health information sent from your physician’s office. Faster, reliable communication is accessible 24/7.

This patient portal is intended to save you time and enhance the flow of communication between you and your healthcare provider. You may contact your doctor’s office by telephone (863-666-3436) at any time. Keep in mind that it will be your responsibility to keep your username and password secure and confidential.

Navigating the portal is made easy with tabs on the left side of the screen for “Home”, “Appointments”, “Messages”, “Billing”, “My Health”, “Tasks and Tools” and “My Profile”.

Security Setting: Please update your password and set up your profile. Fill out all the necessary patient information.

Appointments: This information is needed before your 1st visit:

  • Health History – please fill out all the medical history questions completely.

Please review the following policies, check that you have read and understood the documents, type your name and submit. You may also print a copy of the documents for your reference.

  • Medical Records Release/HIPAA Authorization
  • Financial Policy
  • HIPAA Acknowledgement

Messages: You have an inbox where you can send and receive messages from our practice.

Billing: You can access your outstanding charges and also make on-line payments.

My Health: You can view your medications, care summaries and medical history.

Tasks and Tools: You have access to downloading your health data, learning materials, referrals, and your portal activity.

If you do not complete your patient portal you are required to arrive 1 hour prior to your appointment time. If you have completed your patient portal please arrive 30 minutes prior to your appointment time. Please bring your insurance cards, photo identification, medical records; including x-ray or MRI films, blood work, and nerve tests. Failure to arrive at the designated time may result in your appointment being rescheduled in order to avoid inconveniencing other patients.

Copays and deductibles are payable at the time of service. Acceptable forms of payment are cash, debit/credit card including Visa, MasterCard, AMEX and Discover.

If you need to reschedule this appointment please give us 24 hours notice, or you may be charged a no show fee.

**Requires appointment to be scheduled by the referring doctor. Authorization must be obtained prior to the appointment being scheduled.

Patients requesting electronic copies of their medical records can obtain them free of charge by accessing their patient portal.

Patients requesting paper copies of their medical records must complete a signed release form. The charge is $.15 per page with a request turn around time of 10-14 business days. Records can be picked up with a photo ID; they cannot be mailed. This is to ensure patient confidentiality.

Radiology copies are provided via film or CD depending on the file type available.

Radiology films can be picked up and returned with a signed release and photo ID free of charge.

Electronic radiology copies are provided on a CD with a signed release form and photo ID at $6.50 with a request turn around time of 10-14 business days.
